In compliance with the Affordable Care Act (ACA), Midstate Independent Living Choices provides Form 1095-C to all full-time employees and covered individuals. This form includes information about the health insurance coverage we offered you during the 2025 tax year.
Due to recent federal legislation, we will not automatically mail Form 1095-C to all employees. Instead, the form will be furnished upon request.
